Welcome to the Integrated Health Care e-registration for TISHIP

Please read the instructions below properly before you process with the registration.

1) All students are required to use valid Matriculation Number given to them by the school authority and create a username of their choice.

NOTE: only a valid Matriculation number from the school authority will be accepted.

(2) You will be required to provide a valid email address and a username to create account for registration. Click on new student Registration to proceed.

(3) On successful creation of your profile user account you will be require to login with the username and password with a minimum of 6 characters.

(4) On the login page please use your username and password created to login.

(5) NOTE: When filling the form all fields are mandatory except for the medical history fields.

(6) Upload your most recent passport 130 x 150 pixels in size in JPEG format.  Click here to download a tool for resizing your passport.

(7) Student are required to carefully fill the online registration form and confirm all records before clicking on the submit button. All data submitted cannot be modified or edited after submission.
